I Forgot to Take my Warfarin Tablet | Browns Pharmacy Guide

Warfarin should be taken as a single dose at the same time each day. Although it is safe and acceptable to follow the advice in the PIL, the Warfarin Anticoagulant Record or Yellow Book offers the following advice:

  • If you miss a dose and remember before midnight, you can still take that dose.
  • If you forget your dose for a longer time, do not take that dose to catch up. Instead take your next dose when it is due.
  • NEVER double dose to make up for a missed dose.

Reporting Missed Doses

If you ever miss a dose, you should make a note of the date, preferably in the anticoagulant record book and remember to discuss this with the healthcare staff at your next blood test appointment.
